University Hospitals Geauga Medical Center offers the Norma N. Chapman Senior Adult Assessment Program. This program provides a comprehensive, mental and physical assessment for the older adult. Our goal is to assist older persons, their families and primary care physicians to identify special health care needs and provide optimal, individualized solutions.

Recommendations and follow-up are made with the patient families, primary care physicians and agencies involved. Referrals are made to community agencies, physician specialists and for a functional driving assessment.
